Background
Insulin receptor (INSR) is a receptor tyrosine kinase which is well known for its regulatory role in peripheral glucose metabolism. INSR contain disulfide bridged 135-kD alpha subunits (extracellular ligand binding portion) and 95-kD beta subunit (an extracellular, a single transmembrane, and an intracellular kinase domain). During signaling, ligand binding to alpha subunits activates the intrinsic kinase activity located in the beta subunits and subsequently initiates a cascade of phosphorylation events leading to various biological functions. Unlike other RTKs, INSR's most activities require accessory molecules called insulin receptor substrates (IRSs) e.g. example IRS1-4 is required to engage multiple downstream signaling. INSR kinase can activate two major cellular signaling pathways, PI3K/Akt and Ras/MAPK, which regulate diverse cellular processes including gene expression, protein synthesis, and vesicle trafficking, which result in the regulation of glucose, lipid and protein metabolism, cell growth and differentiation. Defects in INSR have been linked to several pathologies including Rabson-Mendenhall syndrome (RMS), leprechaunism (LEPRCH), noninsulin-dependent diabetes mellitus (NIDDM), familial hyperinsulinemic hypoglycemia type 5 (HHF5) and insulin-resistant diabetes mellitus with acanthosis nigricans type A (IRAN type A).
