Background

IGLON family member 5 (IgLON5) is a member of the IgLON (LSAMP, OPCML/OBCAM, neurotrimin) family of neuronal cell adhesion molecules which, in turn, are part of the much larger Ig domain-containing superfamily (1). The overall structure of IgLON proteins contain three immunoglobulin domains, with numerous potential glycosylation sites, followed by a glycosylphosphatidylinositol (GPI) anchor (2). Mature human IgLON5 is 306 amino acids (aa) in length and shares 98% aa identity with both mouse and rat. Although the specific function of IgLON5 has yet to be elucidated, IgLON proteins are known to form homophilic and heterophilic complexes along the cell surface and several have been shown to shed from mature cortical neurons in a metalloproteinase-dependent manner. These go on to play a role in processes such as neuronal differentiation and survival, axon guidance and outgrowth and cell migration (3, 4). IgLON-5 is expressed in both neurons and oligodendrocytes (5). In the brain, the level of Iglon5 transcript is highest in the brainstem, thalamus and eye, and weakest in the hippocampus, septum, and pituitary gland (6). IgLON-5 is also expressed in non-neural organs and tissue types where it may function as a tumor-suppressor (6-7). Newly classified anti-IgLON5 syndrome is a novel neuronal tauopathy where antibodies target surface IgLON5, causing an irreversible internalization of the protein, manifesting as sleep dysfunction (8-10).
